Denied treatment · Denied coverage · Denied claims
Your doctor said you need it.Your insurer said no.
Health plans deny surgeries, medications, imaging, rehab and hospital stays every day, often with a form letter. Appeals have strict steps and short windows. Miss one and the denial can stick.
Red flags
Signs they’re playing games.
- “Not medically necessary” over your doctor’s written opinion
- “Experimental or investigational” for an accepted treatment
- A prior-authorization denial that delays care
- A claim reprocessed and suddenly denied after you were treated

What we do
How we fight it.
- 01
Figure out what kind of plan you have (employer, individual, government) because the rules differ
- 02
Meet every appeal deadline and build the record the reviewer will see
- 03
Use internal appeals and independent external review where the law allows it
- 04
Go to court when the plan won’t follow its own terms
Questions
Health Insurance Denials, answered.
Does it matter whether my insurance is through work?
Yes. Many employer plans are governed by a federal law called ERISA, which has its own appeal process and rules. Individual and government plans work differently.
Can I appeal a denial myself?
You can, and you should never let a deadline pass. Having a lawyer build the appeal record can matter, because that record is often what a court reviews later.
